    Need guidance on diet or a health condition? Our nutritional counseling service is available at no extra cost.

    JCCGCI also offers Medicaid enrollment help, SNAP/food assistance, rent and utility support, and senior services. Ask your care coordinator what else you may be eligible for.

    Schedule a free assessment to find out if and what you and your family are eligible for. You may qualify due to age, mobility, health challenges, or a hardship.

    Get Approved and Sign Up

    Once approved, we review your information, match you to the services you need, enroll you in a plan for each service, and confirm your delivery details.

    We Prep, You Eat

    Our kitchen prepares fresh, health-focused meals that fit your needs. We deliver them right to your door so you can enjoy healthy, nourishing food every day.

    Frequently Asked Questions

    JCCGCI’s Social Care Network currently provides screening, navigation, and enrollment for groceries and medically tailored meals in Brooklyn, Manhattan, and Queens. If you live in one of these boroughs, call us to find out what you may be eligible for.

    The entire screening, eligibility assessment, and enrollment is generally completed in less than 30 minutes on one phone call. Households with multiple family members may take a bit longer.

    Once you complete your screening and eligibility assessment, your care coordinator will let you know which services you’re eligible for and get you enrolled in those you choose, such as groceries or medically tailored meals.
