JCCGCI’s Horizons Academy is looking to hire a Case Manager who is committed to assisting participants to stay in school and preparing them for life after graduation.

 

The Horizons Academy Program, located at Abraham Lincoln High School, provides academic and motivational support to students, resulting in improved graduation rates for high-risk students from low-income families. If you have a passion for making a difference in the lives of others, this is the position for you!

 

Responsibilities Include:

  • Form positive and supportive relationships with School administration, participants and parents or guardians.
  • Monthly student check-ins.
  • Partner with Director in overseeing monthly programming for current and alumni students (create monthly calendars, monitor attendance, place purchase orders).
  • Supervise program participants in Horizons Academy Tech Lab.
  • Provide workshops on various topics including College Counseling, Team Building, and Service Learning.
  • Conduct Spring and Summer Internship Site Visits.
  • Collect and maintain enrollment documents, student files, monthly progress notes and other required record-keeping and reporting.

Requirements:

  • Bachelor’s Degree required.
  • Proficient computer skills.
  • Strong communication and interpersonal skills for effective participant interaction.
  • Excellent organizational skills.
  • Detail oriented.

Full time, In Person.

$28-30 per hour
