JCCGCI is looking to fill a full-time Case Manager position for a Dept. of Probation workforce development program. Responsibilities will include case management, job and life skills coaching, job placement and workshop facilitation.

The ideal candidate for this Case Manager position will be deeply knowledgeable about and in tune with an high-risk/justice involved population. S/he will be able to create a welcoming, caring, positive, respectful environment that will engender the trust of the target population.

RESPONSIBILITIES:

  • Provide case management services following best practices to each participant
  • Conduct an assessment and create goals
  • Identify barriers to work readiness
  • Make referrals to supportive services and follow up on outcomes
  • Develop participants’ soft skills and life readiness skills
  • Provide work readiness services
  • Assist with job placement
  • Closely monitor every participant’s progress
  • Adjust individual goals on a case by case, day by day basis.
  • Facilitate relevant workshops
  • Maintain contact with participants for up to 6 months post program completion
  • Ensure achievement of gains in individual participant’s skills and attitudes towards work readiness

QUALIFICATIONS and SKILLS:

  • At least 2 years of case management experience including assessment, goal setting, referrals to supportive services and identifying barriers to work readiness
  • Experience providing employment services and life skills including identifying potential resources, assistance with job placement and other basic life skills and work readiness services
  • Outstanding relationship building skills
  • Flexible personality that reacts well to changing and at times unsettling situations
  • Work well one-on-one to identify a participant’s needs, set up an individualized program plan based on those needs, and guide participants toward attaining their individual plan goals
  • Ability to tutor participants as needed
  • Facilitate relevant workshops and training’s
  • Strong coaching and supervisory skills
  • Ability to maintain accurate records
  • Cultural competence

Salary level: Commensurate with experience
